I acknowledge the below terms and conditions required for USL Preferred Vendor approval. *

I understand that a student group representative or USL staff member will provide the vendor a printed and approved Purchase Request Form (PRF, previously FTF) in exchange for food/merchandise. As a reminder, the PRF is an IOU, ensuring future payment from Columbia University’s Undergraduate Student Life department.

I understand that invoices and Purchase Request Forms (PRFs) should be emailed to USLadmin@columbia.edu within 30 days of the transaction to ensure timely payment processing. It is the responsibility of the vendor to retain the approved Purchase Request Form and pair it with the associated invoice to bill the Undergraduate Student Life department officially. Note: Scanning the invoice and corresponding PRF is the quickest way for us to process payment!

I understand that providing inaccurate and incomplete documentation, including invoice and approved Purchase Request Form, may delay payment. Additionally, invoices submitted outside the current academic year may not be eligible for payment.

I understand that continued failure to submit invoice documents within 30 days of the transaction may result in removal from the USL Preferred Vendors List.

I understand that as a USL Preferred Vendor, this arrangement is solely with Undergraduate Student Life, a department within Columbia University. Any invoices unrelated to USL must be directed to the external department's point of contact. USL cannot process invoices for external departments.
