Columbia Access Requirements
- UNI: Your UNI, consisting of your initials plus an arbitrary number, is the key to accessing services and resources at Columbia. Don't wait; set up your UNI now!
- Photo Upload: The University ID card is your passport to Columbia University. It grants you access to the Columbia Libraries, your residence hall, and secure campus locations. Upload a photo for your University ID by Friday, June 5, 2026. Your ID will be ready for pick-up during check-in.
Form Submission Deadline - Friday, June 5, 2026
- Participation Form: Please provide your contact, emergency and other information to help us plan the best possible experience for you during the program.
- Parent/Guardian Consent Form: Please have your parent sign the completed form and upload a scanned copy to our document upload page.
- Conflict Form: Please fill out this form only if you anticipate having a schedule conflict during the Summer Bridge Program. Since full participation in the Summer Bridge Program is a requirement for you to enroll at Columbia University in the fall, we ask that you do not make any major outside commitments that would interfere with the four-week program and require a detailed explanation of any conflict accommodation requests; upon receipt, ASP staff will review your request and determine whether your conflict can be accommodated.
Note: exceptions will only be made for extenuating circumstances, such as a religious observance or an urgent medical concern that may need attention while classes are in session. - Dietary Needs Form: Please fill out this form only if you have special dietary needs.
Health Requirements Deadline - Friday, June 5, 2026
In order to participate in the ASP Summer Bridge Program and gain access to Columbia's campus, you must follow all required University Public Health Protocols:
- Health Insurance: All students are expected to have health insurance coverage during the Summer Bridge Program, which includes emergency care and major medical coverage for hospitalization. If you don’t have health insurance, you can opt to participate in Columbia’s Early Arrival Insurance Plan by signing and returning the Enrollment Form. Should you wish to opt out of the health insurance provided by Columbia, you will need to sign and return a certification of your own insurance. The applicable form should be completed, signed, and uploaded to our document upload page.
- Immunization: All students are required to comply with immunization requirements. Step-by-step instructions on completing immunization requirements for MMR and providing your Meningitis decision via the Patient Portal can be found on the Immunization Compliance website, as well as instructions for requesting a medical or religious exemption. Reach out to immunizationcompliance@columbia.edu with questions.
- Health History: Columbia Health expects all newly admitted students to complete the Student Health History Form. It is completely confidential and becomes part of your medical record. Log in to the Student Health Portal to submit the Health History Form.
- Minor Authorization: All students who will be under the age of eighteen at any time during the Summer Bridge Program must have their parent or guardian sign the Minor Authorization Form and then email the signed form to cuhmedicalrecords@cumc.columbia.edu.
