Welcome! The purpose of this form is to collect important information about your student organization's travel plans. Before you fill out this form, your organization must complete the following steps:
- Meet with your adviser to review your travel agenda
- Ensure your trip is registered and have all travelers complete the Student Domestic Travel Form
- Receive required approval on a Financial Transaction Form (i.e. Governing Board representative, if applicable, and adviser)

Note: You must schedule travel-related appointments with Administration & Planning at least two weeks prior to your departure date. Please plan ahead to allow enough lead time.
Before you schedule an appointment, your group should prepare an itinerary. We also require each traveler's date of birth for all flight purchases. Please obtain this information from all travelers, and please come prepared to share it in person during your appointment. Dates of birth are considered sensitive information and cannot be shared electronically.
